La gerodontologia ai tempi del Covid

Il quadro clinico del paziente anziano odontoiatrico è rappresentato da una ridotta capacità masticatoria e problematiche complesse che lo rendono particolarmente vulnerabile. Polipatologie, difficoltà e ridotta compliance determinano cure protratte nel tempo con conseguente pianificazione più articolata. Come altre specialità chirurgiche, l’odontoiatria è stata fortemente condizionata dalle misure restrittive conseguenti alla pandemia Covid-19, soprattutto per quanto riguarda l’attività programmata. La consultazione telefonica implementata nella nostra struttura ha evidenziato quale criticità l’identificazione dei bisogni reali del paziente che, nella dolorosa esperienza di solitudine, viveva come trauma profondo anche la preclusione dell’accesso alle cure odontoiatriche. L’atteggiamento era tuttavia dicotomico: la ricerca della cura, del contatto sociale si accompagnava alla fuga per la paura e per il disagio interiore con gravi ricadute sullo stato di benessere generale.The clinical picture of the elderly dental patient is represented by a reduced chewing capacity and complex problems that make him/her particularly vulnerable. Polypathologies, and reduced compliance lead to protracted treatments over time with a consequent more articulated scheduling. Like other surgical specialties, dentistry was strongly influenced by the restrictive measures resulting from the Covid-19 pandemic, especially with regard the planned activity. The telephone consultation implemented in our facility highlighted the identification of the real needs of the patient who, in the painful experience of loneliness, also experienced the foreclosure of access to dental care as a profound trauma. However, the attitude was dichotomous: the search for care and social contact was accompanied by an escape due to fear and inner discomfort that can have serious relapses on the general well-being.
