"Health Care and Education in Practice" means addressing the politics of care and health in disease management for the promotion of urban and community welfare. This research analyzes the bidirectional relationship between periodontitis and diabetes, highlighting not only the clinical profiles but, above all, the social, health, and educational implications for improving the lifestyles of individuals in the educational community. Diabetic patients, often frail or socially vulnerable, require ongoing educational programs to increase awareness and self-management skills. The health educator, in synergy with the healthcare professional, aims to develop preventive behaviors, life skills, empathetic communication, and empowerment, ensuring equal access to social and health services, for the construction of community welfare. The biopsychosocial approach to the management of periodontitis in diabetic patients, which integrates clinical, psychological, and educational dimensions, allows for more humane and effective treatment based on an interdisciplinary approach combining health education, inclusive policies, and health education, through a "human-centered" educational challenge.
